How to Apply

Click on the Eligibility Form button below to determine if you are eligible.  If eligible, you will be directed to the application form.

Before you begin your application, please gather the following supporting documents:

  • IRS 501c3 determination letter

  • W-9 form

  • Any supporting documents including photographs that demonstrate your program or idea.

After your proposal is accepted, you will need to file interim and final reports.
